Becoming a Client of Four Villages
Four Villages welcomes individuals who live in our service area.
Our service area extends south from St. Clair Ave. W. to Lake Ontario and east from the Humber River to Roncesvalles. If you live outside of this area, you can visit the Association of Ontario Health Centres to find a Community Health Centre near you, or call us for assistance.
Becoming a Client of Our Primary Care Team (family doctor, nurse practitioner)
We accept clients who live in our service area and are without a family doctor or nurse practitioner in the City of Toronto. An exception is if your family doctor is closing their practice. If you wish to switch for other reasons, please feel free to call us to discuss your needs.
We can see you if you do not have health insurance (e.g. OHIP). However, we cannot accept visitors (anyone with a tourist or student visa) or those with temporary work permits.
Currently we have a wait list to access many of our services at Four Villages. Wait lists are managed to ensure that that those with urgent care needs are given priority.

At present, our social workers are not accepting new clients for counselling. If you need assistance finding another counsellor in the community, Jennifer, our Intake Worker would be happy to assist you.
If you are interested in participating in one of our group programs, please consult our program calendar. Staff responsible for each program are listed, with contact information provided.
If you are interested in a shorter wait time to see a family doctor, please check out Health Care Connect. It is a Government of Ontario program that can match you to doctors or nurse practitioners taking new patients in the community.
